PARTNERING FOR RESULTS PROPOSAL REVIEW PANEL

TEAM MEETING DATES and TIMES:

 

Monday August 15 @ 2:30 p.m.

All  “Supportive Living” Proposals will be ranked based on team score

1.       Catholic Charities – Senior Connections

2.       Community Cooperative Ministries – Meals on Wheels

3.       Deaf Service Center - IMPACT

4.       Dr. Piper Center – Senior Companion Program

5.       Dr. Piper Center – Senior Employment

6.       HOPE Clubhouse – HOPE Clubhouse

7.       ICAN – ICAN Case Management

8.       IMPACT – Early Intervention

9.       LARC – Day Activity Training

10.    LARC – Group Homes

11.    LARC- Community Support Services

12.    Senior Friendship Centers – Case Management

13.    Senior Friendship Centers – Nutrition Services

14.    Visually Impaired Persons (VIP) – Blind Babies

15.    Visually Impaired Persons (VIP) – Independent Living

16.    Visually Impaired Persons (VIP) – Vocational Rehab
