SARF: Space Allocation Request Form

  •  Space Allocation Request Form (SARF) 

  • This form is intended to be used when a Lee County department is requesting minor changes to the physical space / built environment.

    • Minor remodel of an existing space
    • Partial relocation or minor expansion of a department
    • New Furniture
    • Finish changes to include flooring, paint, millwork, ceiling, restrooms
    • Information Section
      • Be as detailed as possible in the information sections of the form. These descriptions tell the story of why the project is needed and what the impact is on the department if the project is not done. This is the department’s business case that will be reviewed by County Administration.


NPR: New Project Request Form

  • New Project Request Form (NPR)
  • This form is intended to be used when a Lee County department is requesting a major project not in the CIP.

    • Major remodel of an existing space
    • Entire relocation or expansion of a department
    • Information Section
      • Be as detailed as possible in the information sections of the form. These descriptions tell the story of why the project is needed and what the impact is on the department if the project is not done. This is the department’s business case that will be reviewed by County Administration.


PCR: Project Change Request Form

  • This form is intended to be used when a Lee County department is requesting a change to a project that is under construction or in design and past the 30% design review.

    • Changes that impact project schedule and/or budget
    • Information Section
      • Be as detailed as possible in the information sections of the form. These descriptions tell the story of why the project is needed and what the impact is on the department if the project is not done. This is the department’s business case that will be reviewed by County Administration.

Consultant/Contractor Evaluation Form

This evaluation form is designed to gather feedback from consultants and contractors regarding the department’s performance and efficiency during the course of a county project. The purpose of the form is to identify strengths and areas for improvement in project management and communication, helping to enhance future collaboration and service delivery.

The form includes criteria such as:
Adherence to Schedule
Effectiveness of Bi-Weekly Progress Meetings
Timeliness and Completeness of Responses to RFIs (Requests for Information)
Accuracy and Promptness of Pay Application Processing
Communication and Coordination with the Project Manager, Engineer, and Architect
Efficiency of Project Closeout Procedures

Participants are encouraged to provide honest ratings and constructive comments to support continuous improvement in departmental operations and project outcomes.
